
Pinhead has a video game now, and it took until October 8, 2026, to happen. Clive Barker's Hellraiser: Revival, from Saber Interactive and developer Boss Team Games, is out today on PS5, Xbox Series X|S, PC via Steam, and Nintendo Switch 2. It's the first video game ever built around Clive Barker's Cenobites. The premise: go to Hell, uncensored, to get someone back.
The setup follows Aidan, who along with his girlfriend Sunny stumbles onto the "Genesis Configuration," a next-generation take on the Lament Configuration puzzle box that's driven the film franchise since 1987. Messing with it pulls Sunny into the Labyrinth. Aidan goes in after her, and the Configuration itself becomes his toolkit: pyrokinesis, telekinesis, and the series-signature Hell Chains all run through it, turning the first-person action around a weapon that's also the reason everything went wrong in the first place.
The casting detail that's driven most of the pre-launch buzz is Doug Bradley. The original Pinhead actor, who last voiced the character in a Hellraiser project nearly two decades ago, returns for Revival, with the studio building marketing around new developer-diary footage of him in the booth.
No cuts for the US release
Saber went into the ESRB process wanting to keep the game as brutal as the films, and the board let them. Chief creative officer Tim Willits told press ahead of launch that the title cleared its ESRB rating without removing anything, landing at Mature 17+ with descriptors for Blood and Gore, Intense Violence, Nudity, and Strong Sexual Content stacked on the rating page. That's a wider spread of descriptors than most horror releases carry, and it matters here because Hellraiser's identity has leaned on body horror and S&M-adjacent imagery that other publishers sand down to avoid an Adults Only rating that would lock the game out of PlayStation and Xbox storefronts.
Willits has said the team is still working through ratings boards outside the US, flagging Australia and Japan's CERO as the harder gates to clear. The American release, as of today, is the uncut version the studio built.
Not everywhere gets the same game
Germany is the exception, and it happened fast. The German Steam listing for Hellraiser: Revival was pulled ahead of launch after the country's USK ratings process stalled. German outlets report that an objection from one of Germany's federal states triggered a second review. Without a valid USK rating, Steam has no legal path to sell the game in that market, digital pre-orders included. Physical retail pre-orders, per the same reports, kept moving.
Multiple reports describe Saber modifying the German build to get through the process: removing the ability to sever limbs from enemies already dead, a narrower cut than it sounds since it targets one post-kill dismemberment interaction rather than core combat. No broader "international" censored cut comes from this change. Reports describe it as a Germany-only build, separate from the version shipping everywhere else today. Ratings situations can change within days of a launch, so German players should check their regional Steam store before assuming either version is live.
